Swansea Bay University Health Board Maternity Services: Legal Support for Affected Families

Independent reports prepared by the Healthcare Inspectorate for Wales and Llais into maternity and neonatal services at Swansea Bay University Health Board have revealed significant and distressing failures in care provided to mothers, babies, and families. What Has Happened and What Do the Reports Say?

In December 2023, the Healthcare Inspectorate for Wales published a report following an inspection of the maternity unit at Singleton Hospital in Swansea. This report identified areas where maternity services needed immediate improvement, and inspectors identified several patient safety concerns.

A further inspection took place in April 2024, and while improvements had been noted, there were still challenges, and further improvements were needed.

As a result of the Healthcare Inspectorate for Wales report, Swansea Bay University Health Board commissioned an independent report into maternity and neonatal services.  This review is ongoing, with the report yet to be published.  Further details of this review can be found here.

The most recent report prepared by Llais  (an independent body set up to give the people of Wales a stronger voice in their health and social care services) in May 2025 identified the following key themes, having heard directly from over 500 people about their journey through pregnancy, birth and postnatal care:

Variation in quality of care and communication

A lack of continuity and involvement in decisions

Inconsistent postnatal support and pain relief

Barriers to raising concerns or being taken seriously.
